How do I apply?
We accept applications in person only and provide two locations, one in Fort Collins and one in Loveland. This schedule will indicate the dates/times and locations to apply. Please note our final day to accept applications is November 19, 2011.
Applicants must provide the following information (**Please note changes):
· A photo ID for the parent
· **Birth Certificate for each child
· Proof of address (for example: electric bill or phone bill)
· Gift ideas for each of your children (less than $20)
If an applicant does not speak English, they are asked to bring along someone who can translate for them.
